On the obverse, an athlete standing on some steps, holding in his right hand a laurel crown, symbol of victory, and raising his left arm. In the background, a bas relief illustrating the sports disciplines from Antiquity. Behind, a Greek temple. Above the figure of the athlete, the inscription "OLYMPIAD" and on the rock bottom right "1904".
On the reverse, the goddess Nike, goddess of victory, standing on a globe. She is holding a laurel crown in her left hand and a palm leaf in her right hand. In front of her, a great crown, with in the centre a space for putting the name of the sports disciplines. Behind Nike, the bust of Zeus on a plinth. The inscription "UNIVERSAL EXPOSITION ST.-LOUIS U.S.A". The design of these two sides was inspired by the medal of the 1896 Athens Games and 1900 Paris Games.
奖牌正面为一名运动员站在台阶上,右手拿着一簇月桂花冠,象征着胜利,左手高高举起。背景图案为浅浮雕,展示古代体育竞赛分项,浮雕后面是一座希腊神庙。运动员图像上面印有“奥林匹亚”字样,右下角注有年份“1904”。
奖牌背面为希腊胜利女神奈基站在地球上。她左手举着月桂花冠,右手拿着棕榈叶。她的前面是一个大花冠,里面写有各个竞赛分项的名称;后面是一尊放置在柱基上的宙斯半身像。牌面题有“美国圣路易斯世界博览会”字样。本届奥运会奖牌的设计灵感来自1896年雅典奥运会和1900年巴黎奥运会奖牌。
